Open the email that has the attachment. Right click once on the thumbnail image located in the message body. On the drop-down menu, select Save as...

By default it'll want to save it in your Documents folder. You can save it that folder or you can save it to your Desktop by selecting Desktop located on the left column.

Click the Save button.
